EDGE Performance Hockey — Release of Liability, Waiver of Claims, Assumption of Risks and Indemnity Agreement

BY COMPLETING THIS REGISTRATION YOU WILL WAIVE OR GIVE UP CERTAIN LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE OR CLAIM COMPENSATION FOLLOWING AN ACCIDENT. PLEASE READ CAREFULLY.

Organization: EDGE Performance and its directors, officers, employees, instructors, guides, agents, representatives, independent contractors, subcontractors, suppliers, sponsors, successors and assigns (collectively, the "Releasees").

Sport Activities: Hockey, including all hockey development activities, events, or services provided, arranged, organized, conducted, sponsored, or authorized by the Releasees.

ASSUMPTION OF RISKS

I am aware that hockey involves many risks, dangers, and hazards and I FREELY ACCEPT AND FULLY ASSUME ALL SUCH RISKS, DANGERS AND HAZARDS AND THE POSSIBILITY OF PERSONAL INJURY, DEATH, PROPERTY DAMAGE OR LOSS RESULTING THEREFROM.

RELEASE OF LIABILITY, WAIVER OF CLAIMS AND INDEMNITY AGREEMENT

I agree TO WAIVE ANY AND ALL CLAIMS and TO RELEASE THE RELEASEES from any and all liability for any loss, damage, expense or injury, including death, DUE TO ANY CAUSE WHATSOEVER, INCLUDING NEGLIGENCE, BREACH OF CONTRACT, OR BREACH OF ANY STATUTORY OR OTHER DUTY OF CARE.

PARENT / GUARDIAN AUTHORITY

If the participant is under the age of 18, I confirm that I am the parent or legal guardian and have full authority to enter into this Release Agreement on their behalf.

EMERGENCY MEDICAL CONSENT

In the event of an emergency, I authorize EDGE Performance and its representatives to obtain emergency medical treatment as deemed necessary.

I confirm that I am the parent or legal guardian of all participants listed above, or that all participants are 18 years of age or older.

I acknowledge that all participants have been advised to wear all protective equipment required by the rules and regulations of the governing body for hockey.

I confirm that I have read and understood the above Release Agreement in its entirety.

I agree to the Release of Liability, Waiver of Claims, Assumption of Risks, and Indemnity Agreement, and I am aware that by agreeing I am waiving certain legal rights.

I authorize EDGE Performance to obtain emergency medical treatment for all participants listed above as deemed necessary.
